This article describes an “At Risk Care Plan,” the goal of which is to improve safety and quality care by anticipating individual safety and care needs of patients at risk and to communicate those needs to the health care team. This intervention has reduced hospital readmissions, costs, and adverse events for high-risk adult inpatients in a small community hospital.
The purpose of this article is to describe an "At Risk Care Plan."
