Joint Commission requesting VTE discharge instructions

The Joint Commission is asking hospitals to share their discharge instructions and educational materials for patients with venous thromboembolism (VTE) who are on anticoagulants.

The Joint Commission’s Department of Health Services Research is soliciting this information as part of a research project. These materials may be used to develop of a free educational toolkit to assist organizations in developing and/or refining their own VTE materials.

Discharge Instructions for Venous Thromboembolism (VTE): A Comprehensive Approach to Medication Management. The Department of Health Services Research is seeking examples of patient discharge instructions / educational materials for VTE patients being discharged on anticoagulant medications. This solicitation is part of an externally funded research project.
